Skilled Nursing-
The nurse provides skilled services, supervises other team members,
re-evaluates and updates the care plan, and communicates with the
client’s physician. Home infusion, wound care, and ostomy care.
Home Health Aides-
Home Health Aides services include bathing, personal care, meal
preparation, assisting with activities for daily living, and light
housekeeping duties.
Therapies-
Physical Therapy – Services are delivered by Licensed Physical
Therapists in accordance with physician orders. Rehabilitation of
the patient is a key element of home care. Anodyne Therapy.
Speech Therapy – Available with physician orders for rehabilitation
of speech and swallowing orders.
Occupational Therapy – Rehabilitation services are available
with physician orders.
Medical Social Services-
Social Workers evaluate the client’s needs, consult with physicians,
and refer to other community agencies or programs for additional
services.
HomMed® Telemonitoring-
Agency based monitoring of clients seven days a week from the comfort
of your home. Encourages patients to be independent.

Emergency Personal Response system. Agency program-standard of care
for our clients.
